BASIC LIFE w/ accidental death & dismemberment (IAPMO PAID)
IAPMO offers benefit eligible employees a $50,000 basic term life insurance policy with an accidental death and dismemberment rider of $50,000. The benefit has an age reduction of 65% at 65, and 50% at 70.

Life Insurance
voluntary term life w/ accidental death & dismemberment (employee PAID)
Employee Benefit Amount: In increments of $10,000 up to the lesser of 5x annual salary or $500,000
Benefit Reduction Schedule: for employees, age 65 benefits reduce to 65%. Age 70 benefits reduce to 50%
Spouse Benefit Amount: In increments of $5,000 to a max of $250,000, not to exceed 50% of the employee's coverage amount. Spouses are eligible up to age 70.
Unmarried Dependent Children: Under age 26. Benefit amount $5,000 or $10,000
IAPMO offers it's eligible employees an option to enroll in voluntary term life benefits with New York Life. When an employee enrolls, the spouse and dependent of the employee then become eligible. Upon hire, the employee is eligible for a guaranteed issue of the benefit. After the initial benefit period (at open enrollment or during a life event) an employee who chooses to enroll or increase their benefit amounts approval will be subject to medical review (underwriting) by New York Life.
Guaranteed Issue:
Guaranteed Coverage amount is the amount of coverage you can elect without answering any medical questions or taking a health exam. Guaranteed Coverage is only available during Initial Enrollment. If you apply for coverage that is above the Guaranteed Coverage Amount, or if you are applying for coverage after 31 days after you become eligible, you must fill out a Medical Evidence of Insurability form. All dependent child benefits are guaranteed issue.
